Here are some of the ways to earn points for your Wellness Book that relate to preventing future health-related issues.

1.  Employee Health Fair with Know Your Numbers
(50 points/year with labs, 20 points/year without labs, 10 points/year if spouse attends)
2.  Wellness Wednesdays
(5 points per visit)
3.  Receiving your yearly flu shot
(5 points/year)
4.  Having an annual physical exam
(5 points/year)
5.  Completing an annual dental checkup
(5 points/year)
6.  Receive a vision exam
(5 points/year)
7.  Mammogram/prostate exam
(5 points/year)
8.  Colonoscopy
(5 points/year)

Talk to your doctor about which health screenings are appropriate for you. Here are some suggestions/options:

20-29    BMI (each regular visit or with Christine), Blood Pressure (each regular health care visit or with Christine every year if BP is greater then 120/80 HG), Lipid Profile (every five years), Clinical Breast Exam (every three years), Pap Test (yearly)

40-49       BMI (each regular visit or with Christine), Blood Pressure (each regular health care visit or with Christine every year if BP is greater then 120/80 HG), Lipid Profile (every five years), Clinical Breast Exam (yearly), Mammography (every one to two years), Pap Test (every one to three years, depends on type of test and past results) Blood Glucose Test (every three years at age 45 and older)
